   A clarification question: You are basically asking for something that, on a one-sheet 18″x24″ paper, will with a quick glance allow the person to tell what moon phase it is on a given day during the 366 days in 2012?

   If so, I assume then you’re looking for something fancier than the ones that many planetaria sell with a grid showing the month on the top, day on the side, and the moon phase in the grid formed by those?

   Not necessarily more elaborate than a traditional grid calendar, just a more elegant solution to the problem of communicating the information.
